- City Representation: Indicate the city you will represent during the tournament.
- Player Information: Provide your name, age, and contact details.
- Jersey Selection: Choose your preferred jersey size from our available options to stand out on the court. Jersey numbers will be distributed on a first come, first serve basis.
- Tournament Preferences: Select your preferred tournament dates and times.
- Registration Fee: A $300 fee is required to secure your spot in the tournament. Payment details will be provided upon completion of the form.
COMPETE FOR CASH PRIZES:
